My 5 month old baby's soft spot is raised and pulsating.

In what position?: It is common for the soft spot to feel full if a baby is laying down & a bit less full when they sit up. Pulsations would be odd in any condition & worth a doc visit.

Yes: A newborn's soft spot can often show slight pulsation. It is nothing to worry about. As long as your baby is otherwise well you can ignore this.

Does the presence of a bulging fontanel (soft spot) mean that my baby has meningitis?

Possibly, but...: A bulging fontanel is associated with meningitis. But babies with meningitis have other issues, which may include fevers, poor feeding, lethargy. And bulging fontanel is not only caused by meningitis either. Please see your child's pediatrician if you are concerned.

Yes: The soft spot is are region of the skull where the bones are not yet developed but will grow in with time. Major blood channels are located in this region, and the area will pulsate with the heart beat and with breathing in a normal manner. In cases of severe brain pressure the soft spot can bulge and the pulsing may stop. This becomes a serious problem that needs urgent evaluation.

What is the best treatment for delayed closure of anterior fontanel soft spot?

Patience: Most kids will have what feels to be closure of the anterior fontanel by 15-18 mo but some may persist. The separation between the bones will persist to some degree until about 9 when kids reach their adult head size. In examining thousands of kids over 3 decades, I never found one that didn't eventually close.

Probabaly not: This is probably not a vitamin deficiency. That type of pain is more persistent and burning often. It sounds like a nerve impingement. Look at your shoes and perhaps try some inserts in your shoes to give you more cushioning. If no relief see a podiatrist.

Fontanelles: Anterior and posterior Fontanelles, along with skull sutures, are present to accommodate the rapidly expanding brain volume in very young children. Premature closure of these structures results in microcephaly and is cause for concern. Your pediatrician checks these structures at every well visit until 2-3 years of age.

Anterior fontanelle: Should be easy to feel by gently touching with the soft part of the fingertips whether wet or dry. It usually closes at around a year of age, but the timing is quite variable.

Couple possibilities: Increased intracranial pressure, either due to pseudotumor cerebri, systemic hypertension, with Valsalva exacerbations. Sounds like you may have scotoma, or - very unlikely - localized occipital cortex kindling. Are you on depakote for seizures or vascular migraine prophylaxis? Are you on HCTZ (hydrochlorothiazide) for high blood pressure or PTC? Consider stopping Ovcon - associated with bad cerebrovascular events.

Occipital neuralgia.: Irritation of the main nerve that runs from the back of the skull can cause occipital neuralgia.. It is suspected that tense muscles or ligaments may press against the nerve, causing irritation, inflammation and subsequent pain. Other causes may include viral infection, trauma to the neck or bad posture.

Pressure monitor: The soft spot reflects the pressure of the fluid underneath. When a baby is upright, it tends to flatten or dip, like a water balloon. Likewise, when they lay down, it fills and may bulge like the same water balloon. If they cry, pressure goes up, if dehydrated, it goes down. If it is ever tense and full, a doc should check baby.

Having a headache (occipital area) for 2weeks now. Always on the same spot, and feels like a throbbing pain that goes on and off. What causes it?

Occipital neuralgia: This is likely occipital neuralgia and pressing/tapping on the specific spot in the back of your head (occipital nerve trunks) may cause local tenderness, shooting pain, tingling along the back of the head and is supportive of this dx if present. Occipital nerve block helps to diagnose and treat this condition. Another possibility is cervicogenic HA (referred pain from the upper cervical joints).

Check for redness: Or sore or lesion could be a parasite, see dermatologist if so, if not could be a nerve irritation, if does not go away see a neurologist
