Any way to fix an awkward gait?

Varies by cause: The key to fixing an awkward gait is determing the cause - is it muscular, skeletal, neurological, etc. Pt can help for muscular injuries, and for some neurological conditions meds can help. As always good diagnosis begets good treatment and better outcomes.

Upright Motion: Gait is the way locomotion is achieved using our limbs. Gait is defined as "bipedal, biphasic forward propulsion of center of gravity of human body, in which there is alternate movements of different segments of the body". Different gaits are characterized by differences in limb movement patterns, overall velocity, forces, kinetic and potential movement.

Can gait still improved at the age of 21?

In general...yes: If you have inefficient biomechanics, your gait can be improved by modalities like physical therapy and orthotics. Your gait is a complex series of coordinated muscle movement and can be influenced by age, structure, muscle control, etc. And affected by functioning body systems (i.e. Nervous system, circulation, musculoskeletal system, etc.). Kind of a tough question without a physical exam.

Ataxia: This can also be called ataxia, difficulty coordinating movements, resulting in unequal jerky movements.

How would a pt perform a wheelchair transfer using a gait belt?

Gait belt = safety: There are many techniques to help a person with debility transfer from/to a wheelchair. It really depends on the level of debility and the amount of help you require. A paraplegic patient may not be able to assist much and may require a slide board where as a hemiplegic may be able to stand pivot with assist. The gait belt is primarily for safety. It allows a steady grip hold while assisting.

No: There are standards that have been established. Normal is considered to be within two standard deviations of the mean, for the most part.

If level ground: Just sort of common sense, uneven terrain can create a greater risk of falling and injury. Maybe, use assistive device or hold onto a companion until you stabilize. Prepare for your hike by getting some gait rehabilitation with a good physical therapist.
