Dose based on weight: In pediatrics, med dosing is usually based on weight. For a simple pneumonia, the Azithromycin dose for 25 kg child would be half the adult dose: 250 mg for 1 day, then by 125 mg daily for 4 more days. Augmentin (amoxicillin and clavulanate) dosing would also depend on weight and the specific illness being treated. OD on antibiotics: There have been recent reports to suggest that for community acquired pneumonia, a single primary line of antibiotics such as Amoxicillin is sufficient. Azithromycin, preferred for its shorter duration has limited efficacy. Tonsillitis: Why did your pediatrician prescribe two antibiotics? This is not typical or advised. Do not over does the child. Call your doctor and clarify this. For typical strep or staph tonsillitis, 7-10 days of Augmentin (amoxicillin and clavulanate) every 12 to 8 hrs depending on the dose is enough. I don't see any benefit to adding Zithromax. Sinusitis: Hello, you are being treated in an appropriate pathway. Personally, I recommend a quinolone antibiotic such as Levaquin (levofloxacin) or Avelox after failing the previous two antibiotics. I also add a steroid by mouth to this regimen. Allergy testing may be helpful. You have had ear infections as well. Finally a ct scan of your sinuses would help, especially if you fail another abx. Know What To Trreat: It has not been my experience that the penicillis are really too rough on the intestinal system. Add a matter of fact the azthromycins have a worse reutation. My concern, is it the medications causing the GI symptoms...........or...... could the G I be part of the illness? Since I do not know why you are taking what you have been prescribed it is difficult to comment, but taken together these agents are likely to alter your bowel flora significantly. If antibiotics is effective, patient will feel better in irritative voiding and backache with 2-3 days after its use. But, unfortunately, many patients being given antibiotics for backache with no real proof of uti. So, check with an expert if you indeed have kidney infection by taking a good history and examining a correctly-collected urine specimen.

If the infection is bacterial, there should be no fever greater than 101 by day 2 of 3 of the antibiotic course. I would wait another day before contacting your PCP.
If there is any respiratory distress, an ED visit is suggested.

Resistance suspected: Some germs 'don't respond' to an empiric course of antibiotics. You need proper evaluation with a urinary culture. That will give a 'road map' for what antibiotics can be used... Sometimes there's more to a male UTI compared to a female urinary infection.
