Many possibilities: Throat clearing is from gastric reflux, post nasal drip, too dry saliva, retained throat secretions because the pharynx can't clear them, pharyngeal diverticulitis, foreign body sensation from globus, habit, chronic nasopharyngitis, and others. No and yes: A post nasal drip will not cause asthma but it certainly could cause you to have to clear your throat. Everyone has a post nasal drip and it is only a problem when there is a change in either the quality or the quantity of the mucus. Hard to say: You have not said which inhaler, and there are several types. The overuse of inhalers is a clinical call, based on frequency of use/refills. If you are using it more often than needed or need it more often than expected your treatment plan needs to be updated. Overuse of beta agonists can lead to loss of effectiveness. Your ability to clear secretions is impaired directly from smoking and you are most likely having to clear your throat more frequently because of that reason. That and others: Sudafed can help but try not to use it for a prolonged time. There are anti-allergy meds that can help also. I find nasal saline irrigation to helps me and my patients. Seen ENT: If PPI twice daily failed after several weeks, it is time to see an ENT at an university center. Your current symptoms are not typical for asthma. In children, throat clearing may be a sign of asthma however. With a past history of severe asthma, I think a Lung function test is in order as well. ...Read more

You could still: Have pneumonia. If your chest xray was taken several days or weeks back, it may not have detected signs of pneumonia yet. If the treatment you were Rx isn't working you should really go back to your doctor for another complete exam.
