Heart Involvement: New onset of swollen feet and ankle in someone with ipf is suggestive of cor-pulmonale / right heart failure. This suggests that the lung disease is probably worsening and hence leading to extra strain on the right side of the heart which is now beginning to fail.

You need to see MD: The symptoms you describe are clearly not normal. The potential causes are numerous, and if not identified may result in significant medical problems ranging from loss of function and joint destruction to even cardiac issues. See a rheumatologist: If the problem persists for more than a month, it may be an inflammatory or autoimmune process and should then be seen by a rheumatologist. Until then, Ibuprofen and or Naproxen will be helpful to reduce the joint pains - don't forget to take with food and follow directions for the over-the-counter remedies.

Possible: Common flu symptoms include fever, headache, body aches, joint pain etc. When the symptoms initially develop it may be possible to feel pain in a single joint. If the pain is due to flu the pain my progress to other joints.

Here are some. ..: If just happening overnight, one has to concern about DVT (deep vein thrombosis) although it's uncommon for bilaterally simultaneous occurrence. So, watch such swelling closely for potential progress, go to urgent care timely, & don't wait until the last minute. For the time being, wear compression + leg elevation will help, but not knowing its underlying causes. Best wish. ..

Sciatica?: In my experience, "SI joint" and "Piriformis" syndromes are commonly over-diagnosed by mistake. Instead, I do see a lot of cases of sciatica from degenerate discs and pinched nerves in the lumbar spine. From your description, I wonder if you might have a pinched L5 nerve in your lumbar spine.
