Look for reviews: Sometimes yes, sometimes no. It is better to go to websites where one can read actual reviews written by patients. On HealthTap, one can go to each doctor's profile page and read his answers (to see his "speaking" bedside manner) and read reviews about him by patients and other doctors. No: Insurance companies don't mandate language, but rather that certain components of an exam or history be present in order to bill at a certain level. Using the icd codes translates a diagnosis into a computer code... Most notes have the diagnosis plus the code.
