More info needed: It would be helpful to know your age. A 27cm ovarian cyst has a low chance of being cancer in a young women in her 20s; however, in an women in her 60s it would be very worrisome for cancer. No matter what your age, you should have it surgically removed. Multiple issues: Nabothian cysts are normal as is (probably) the rt ovarian cyst. The uterine polyp is more worrisome and should be followed (with ultrasound) to make sure it doesn't get bigger. Ovarian/paraovarian: The report suggests that the cyst is likely coming off of the left ovary. Exophytic means that it is not contained entirely within the ovary, so not 100% sure it is arising from the ovary. Paraovarian cysts can also be seen arising in the broad ligament between the uterus and ovary. No: It's hard to tell how much pain your ovarian cysts and fibroids cause. An ultrasound exam should be able to tell how big they are and if they are causing any symptoms. Surgery vs. Observe: Any post-menopausal woman with a complex pelvic cyst has to be careful for ovarian cancer; aspiration would be the last treatment option i'd consider unless you have morbidities that prevent you from having a safe surgery, i.e. Morbid obesity, cardiac failure, etc. I would either f/u w/ imaging in 3 mo's for growth of this cyst (unlikley) or accept surgical excision. Not likely: Treatment would be a laparoscopic cholecystectomy. The gallbladder stores bile and pushes it into the intestines in response to eating fat in a meal. Bile is a salt in solution.
