Seems high: For a severe sinus infection I would prescribe 45 mg / kg /day divided in 2 doses. Your child is 40 lb which is 18 kg. Sinusitis: You have either been misdiagnosed or inadequately treated. Under medication, you only report Ibuprofen and don't even list amoxicillin.
