Nothing specific: The anti gad antibodies can be seen in stiff person syndrome, rasmussen encephalistis as well as in many patient is type 1 diabetes. Rarely, stiffman person syndrome can be found in patients with breast cancer and hodgkin's lymphoma. But your symptoms are not consistent with sps. I will not rely heavily on the antobody test alone. Can cause diabetes.: It seems these autoabs can cause stiff man's syndrome, a neurological disorder with impaired motor function and muscle stiffness and spasm. There is also a great deal of glutamic acid decarboxylase in the pancreas and so these antibodies act here and produce diabetes mellitus in the patient. My gad-65 antibody level is >1:4800 rather than the u/ml measurement i see quoted online. How do I convert this number and what does it mean?

Hard to convert: The >1:4800 means they can dilute the serum from your blood more 4800 times and still detect the antibody in their test. Because of differences in how each lab performs the dilutions, runs the tests, and defines a "unit per ml", there isn't a good way to directly convert the two. However, the >1:4800 describes a very large amount of the antibody in your system. See your doctor for more info. ...Read more

When patient has autoimmune flares that cause weeks of glucose intolerance and diabetes-type symptoms that resolve, would it be useful to test for antibodies to gad, insulin or islet cells?

Possibly: Autoimmune diseases tend to occur together, so checking for antibodies for type 1 dm can be helpful to assess for that diagnosis. Glucose tolerance should not fluctuate with a flare in gad/insulin/islet abs. It it more likely to be from treatment of the autoimmune flare (steroids) or stress from the flare. ...Read more

A few comments: Yes, lupus cerebritis can occur. Spinal fluid measure of P-ribosomal protein might help. Your myoclonic seizures could be Juvenile Myoclonic Epilepsy, and could benefit from Depakote or Keppra (levetiracetam). Your emotional state could be assessed by psychiatry. Recommend you see a neurologist, and have all of this looked at thoroughly. ...Read more
