Caseating granulomas: This sounds like tuberculosis, but may conceivably be systemic fungal infection like Histoplasmosis, rarely lymphoma or sarcoidosis. Injections w/ PT: Sorry to hear about your pain. If conservative treatment had already failed, i would consider epidural steroid injections along with physical therapy. It may free you up to get more out of pt. Avoid high velocity manipulation w those osteophytes. Also avoid surgery as long as possible.
