In general, once bone healing is complete - hardware is redundant. Removal of hardware should be based on symptoms...Continued pain, swelling, failure of hardware, allergy to hardware, etc...

See you doctor: First of all, if you are 40 and had surgery 12 years ago the growth plate in your ankle was long closed. My recommendation would be to see an orthopedic doctor and see why and where the pain is coming from. Obviously after 12 years the original surgeries are healed. It: It sounds infected. With a crush injury often the blood supply to the area in question is damaged. It is not usual for an implant that is supposed to be under healed soft tissue to be exposed. There are different reasons why the screws were so easy to come out: little bone for the threads to bite on, poor bone stock, screws too short, perhaps you bore weight on the anke prior to when recommended. The numbness could be from the crush injury, from poor blood supply or from other causes of swelling like having your leg in a down position. ...Read more

Discuss with surgeon: Might I recommend Sitz baths. When I was in practice I charged $10.00 for the hemorroidectomy and $1000.00 / day to rent the Sitz bath :)
Also Fentynl lolly pops and Duragesic patches are very effective.

Without knowing more about whether you have pain at your ankle or pain at your calf or other issues going on - and I can't see or examine your ankle - it's not clear if this swelling related to impaired venous/lymphatic drainage which can happened after surgery in that area, or if it's related to intrinsic joint issues or even a blood clot.
