Treat it: The positive H. pylori antibody test is not a reliable indicator of current infection. The gold standard for H. pylori treatment is the gastric biopsy, and this is negative in this patient. However, I am impressed by the positive breath test and erosive gastritis. I would have expected stool antigen to be positive, as it mirrors the breath test. Lab Error: I would chalk this up to lab error, but in my patients we do not leave it there. There is a much better and more valid test to be done called a western blot test for herpes. I am not familiar with other substances outside of the cannabinoid class of compounds (some of the newer synthetics?) that will react with standard immunoassays. Yes: There are 2 reasons for this kind of result: (1) a false positive, or (2) early infection before seroconversion. In someone at low risk for HIV without symptoms, another negative WB in 1 month means there's no infection. In someone at risk or with symptoms of acute infection, a viral load can be helpful.
