Full bladder US?: If the ultrasound was done with a full bladder as is usually done, then the pregnancy will distort and displace the bladder at 18 weeks, so it is difficult to say that the "bladder was enlarged". A stressed infant will shunt oxygenated blood to more vital organs, less renal perfusion, therefore less urine produced. Anything that decreases placental perfusion can cause it (abruption or placental separation, poor implantation, vascular problems, clots, uterine abnls, etc). Baby full bladder: Most likely if the kidneys look normal and the amniotic fluid is normal, all is well. Usually we'd repeat the ultrasound again later to see if the baby emptied in the meantime or not. You say you have cirrhosis and it is no surprise if you've got a bit of extra conjugated bilirubin in your blood spilling into your urine.
