Symptomatic therapy: Just like many other causes of primary dementias, there is no definitive therapy or cure. However, some medications like donepezil and rivastigmine can help with the memory loss by slowing down the rate of progression of symptoms and allowing patients to maintain certain level of cognitive function for a longer period of time. They also help with some of the behavioral features.

Lewy body dementia - does anyone know of any treatment for this disease?

Few options: There are currently no fda-approved treatments specific for lewy body dementia (dlb). But given the severe cholinergic losses that occur in dlb, the frequent co-occurrence of alzheimer's disease pathology, and several small trials that suggest their effectiveness, cholinesterase inhibitors are the off-label treatment of choice. Exelon (rivastigmine) patch has shown some effectiveness for the cognitive decline.

Good Rx Available!!: Dopamine meds for physical parkinsonism: Azilect (mao-b selective inhibitor) & sinemet +/- Comtan (stalevo (carbidopa and levodopa and entacapone) is both together). Tailor rx watching for side effects. Avoid Dopamine agonists (requip xl/mirapex er/neupro)--too side effect prone for lewy body patients. For dementia: namenda, paired with Exelon patch or aricept. For psychosis: seroquel or even clozaril. For excess sleepiness: nuvigil.

Donepezil effectiveness for vascular and Lewy body dementia?

Helpful in LBD: I routinely use donepezil effectively in my lewy body disease patients. Although only 40-50% of them actually have cognitive improvements in the here & now, but we suspect it slows the progression of cognitive deterioration anecdotally. Can't comment on personal experience with vascular dementia.

But with early dementia, often with associated psychosis (visual hallucinations, delusional thoughts), excessive sleepiness, depression/anxiety/apathy, & rbd. Rbd = rem behavior d/o, causing acting-out of dreams in rem sleep.

Lewy Body Dementia: Lewy body dementia (dlb) is a progressive brain disease with cognitive, behavioral, and motor system deterioration similar to that seen in parkinson's disease. Features include visual hallucinations, a fluctuating course, disorientation, and executive and visuospatial dysfunction with relatively spared language & memory function. Apathy, irritability, depression, and agitation are also common.

Unfortunately no: Lewy body is like alzheimers with progressive loss of cognitive function. It however has more psychiatric symptoms early on including: depression, paranoia and hallucinations. Treatment with some of the alzheimers meds, namenda, (memantine) excelon and arecept may have temporary benefit, but no cure.

Up fluid intake.: Fluid intake is always important, especially in seniors. This is likely the best thing to do here as anything invasive might make things worse. This man is very old and needs to be treated gently. If he is on an iv, he can get fluids that way. If blood pressure gets lower it would be wise to get to the hospital. Best of luck.
