I have a herniated disc at l-5 s-1 with an annular fissure sacroillitis in both si joints and coccyx pain. I have had this pain for over a year have had a surgery to remove a pillion ideal cyst which wasn't there.Have had 3 injections including a coccygea

These : These can be difficult problems to fix and often poorly respond to treatment including surgery. Patients that have failed conservative treatment you may consider the following options:

1. L5-s1 disk with annular tissue - recommend discogram to determine if this is the cause of the pain followed by either endoscopic discectomy or fusion surgery. Please see my health guides on endoscopic disk surgery.
2. Si joint pain - consideration for si joint fusion.
3.Coccyx pain - if pain is related to prior fracture sometimes surgical removal of the coccyx may give pain relief

i would recommend seeing spine surgeon. Good luck! ...Read more

Glucosamine: I would recommend glucosamine sulfate and msm 1500mg per day for joint based pain that is related to articular cartilage wear. Herniated L5/S1 w/sciatica & left SI joint pain for 2 mos. Drs want discectomy. How likely would PT and time make nerve and joint pain go away?

Depends on the case: Sometimes PT and other conservative therapy is enough to fix S1 radiculopathy. It's common for this to be recommended. If your MRI film shows obvious impingement, it may be therapy will not help though. When walking is affected, the reflexes are absent (ankle reflex here) and there is a lot of pain, if the MRI shows herniating disc material it is probably better to have the procedure. ...Read more

Herniated L5/S1 w/sciatica. Would microdiscectomy surgery help with SI joint pain or just the sciatica?

Most likely: just the sciatica. SI joint pain is an arthritic phenomenon, and releasing pressure on the nerves wouldn't affect that. Be sure to speak with your surgeon, as surgery is usually a last resort, and is not always successful in relieving the pain. ...Read more
