These can be difficult problems to fix and often poorly respond to treatment including surgery. Patients that have failed conservative treatment you may consider the following options:
1. L5-s1 disk with annular tissue - recommend discogram to determine if this is the cause of the pain followed by either endoscopic discectomy or fusion surgery. Please see my health guides on endoscopic disk surgery.
2. Si joint pain - consideration for si joint fusion.
3.Coccyx pain - if pain is related to prior fracture sometimes surgical removal of the coccyx may give pain relief

Glucosamine: I would recommend glucosamine sulfate and msm 1500mg per day for joint based pain that is related to articular cartilage wear. If you can get a micronized chondroitin sulfate this can help as well. This can also be helpful for maintaining your vertebral disc health. Depends on the case: Sometimes PT and other conservative therapy is enough to fix S1 radiculopathy. It's common for this to be recommended. If your MRI film shows obvious impingement, it may be therapy will not help though. When walking is affected, the reflexes are absent (ankle reflex here) and there is a lot of pain, if the MRI shows herniating disc material it is probably better to have the procedure. ...Read more

Depends on: Location/past history. If you have pain in a single weight bearing joint (knee/hip/ankle) or in a joint which has a history of a prior injury then it's most likely Osteoarthritis. If there is pain in many of the small joints in your hands and feet then it could be a type of inflammatory arthritis.
