Tough question..: Simple question, but not easy to answer. It is easier to prevent acne than treating severe acne because severe/cystic acnes will cause scars which are permanent. Inflammatory acne are often treated with topical benzoy/antibiotic combo or oral antibiotics. Black-heads acne probably better treated with some retinoid derivatives. Acne: Acne can occur on the scalp. We also sometimes call it folliculitis--inflammation (or occasionally infection) of hair follicles. Doxycycline is a commonly prescribed antibiotic for both acne and folliculitis. Another course of the same could be done and then followed by a topical antibiotic solution like clindamycin. There are no good options for otc treatment--shampoos, etc. Rarely allergic reactions to medications can present as pustules in the scalp. Why do you ask?: Teresa, this is the 21st century and you should not have to suffer with this kind of acne. Exact definitions vary, but "many pustules", especially on a female, is an indication for the isotretinoin you're taking. Can be normal: Within the first month of starting Accutane you can have flare up, which is why sometimes doctors will prescribe a lower dose than your "goal" dose for the first month to limit the initial "flare ups". Some options...: For treatment of acne, over-the-counter benzoyl peroxide gel/cream is quite helpful. If desired, a doctor can add an antibiotic gel/cream such as Cleocin (clindamycin phosphate) gel. If more help is needed, prescription retinoids such as Retin-A or Differin gel can be used instead of benzoyl peroxide. Oral antibiotics such as tetracycline can be used also.
