How effective are second Accutane courses compared to first courses? Should a higher cumulative dose be used the second time around? 18 y/o, m

Probably less: The response rate is probably a bit less for a second course, but its usually still very worthwhile to try, unless you just didn't do well the first time. I use the same routine for the second course, which for me is 1mg/kg daily dosage, usually divided into bid dosing with a total goal range of 120-150 mg/kg.

Yes: Starting acne treatments can initially cause the acne to worsen for 2-4 weeks before it actually improves. Normal course of Accutane is 4 months or longer. It's important to stick with a treatment for at least a month or more to give a fair assessment. Stop treatment if getting serious side-effects. Needs to be monitored by doctor with lab work.

Pretty safe: Accutane has some issues, especially birth defects if taken while pregnant, but most of the other popularized "dangers" are significantly overblown. Still, it must be prescribed by a doctor with experience in its use, and you must join a registry (ipledge) as an added precaution against pregnancy.

How good is accutane?

Very good: Accutane is very drug useful for treatment resistant acne. However, there are several restrictions on its use. Patients need to be fully informed prior to taking the drug and agree to using at least two forms of birth control. This drug is not for everyone and can only be prescribed by physicians specifically trained in its use.

Rarely: It can, but pretty unusual (about 1% of patients experience eye symptoms such as impaired vision, corneal opacities, dry eyes (which make contact lenses uncomfortable), and decreased night vision (among others). How can Accutane be dangerous?

Mostly birth defects: The birth defect risk is quite real; you cannot get pregnant while taking it. But with all respect to previous answerer, other dangers have been overblown, and no cause-effect has ever been proven for many of them. That doesn't mean they never happen, but in 30+ years of prescribing it I have never seen any of them. I agree it should only be prescribed by a dermatologist well trained in its use.
