Depends on how much difference. Some 1-2 cm differences can be present normally, some persons are born with a smaller kidney and some persons have reduced blood flow to the smaller kidney, and some have reflux during childhood.

Not sure: We don't normally describe the kidneys as "bulky" or not bulky. It could simply mean that one kidney is larger than the other in which case in and of itself is clinically insignificant or if one kidney is "bulky" in a part of it then that raises the possibility of a tumor or if the calyceal system is enlarged then there maybe obstruction.
