Perhaps 2 issues: Not unique to have trauma to shoulder, with possible rotator cuff tear, and associated injury to the brachial plexus. But severe abrupt non-traumatic shoulder pain associated with numbness and/or weakness in the arm may be due to brachial plexitis (parsonage-turner syndrome), and should be managed with steroids. You need an : Evaluation to see if there is a nerve root compression in the cervical spine leading to your symptoms which may be managed non operatively or operatively depending on the clinical or examination findings and how they match imaging studies of the cervical spine with possible electrodiagnostic studies being indicated as well if imaging is not definitive. If they happen during a hectic period in your day, like at work, etc., i would say anxiety is likely. MRI with gadolinium: I'd consider MRI with gadolinium contrast of brain, cervical, & thoracic cords. I'm not convinced the pain is muscular based on the information. Doesn't sound like there's testing or examination data of spasms or muscle enzyme elevations, etc. Pain may be coming from the spinal cord or brain.
