Plaquenil (hydroxychloroquine): Plaquenil (hydroxychloroquine) has many side effects. Some common ones include dizziness, abdominal pain, headache, nausea, diarrhea, itching, and sensitiviy to sun. More serious side effects which are more rare include certain types of anemia, muscle pain, and liver problems, among others. Despite the fact that what you describe is not officially under it's side effect profile, it still could be causing your symptoms.

Anti inflammatory: Plaquinal is used in cases of inflammatory arthritis such as rhiematoid arthritis and lupus because it has strong anti-inflammatory characterics. It certainly has a significant side affect profile as well. Presumably your rheumatologist thinks it is indicated for your OA to treat inflmation. Don't hesitate to discuss this with him and never hesitate to get second opinion.

Hard to say: Rashes are common with lupus but are also seen with allergic reactions to ibuprofen. Have the rash evaluated. If you prove allergic to Ibuprofen you will also be allergic to Naproxen so stay off of both until your doctor evaluates the issue.

Need for evaluation: It is not feasible to provide a meaningful opinion without examining you and your symptoms warrant evaluation by a physician. You seem to have an auto-immune inflammatory disorder and it requires more personal care than is feasible to provide in this forum.

It can: Plaquenil (hydroxychloroquine) is not the FIRST line of immunosuppressive medication used for pericarditis. The usual trio is colchicine, a non-steroidal anti-inflammatory and/or prednisone.But if any of these or combination is not helpful, Plaquenil (hydroxychloroquine) can be in your doctor's armamentarium to try.

NAC: I assume you are taking mtx and plaquinil for their anti-inflammatory effects for psoriasis? This combination is also used for a variety of other systemic diseases, too. Nac has been touted by naturopaths as an effective supplement to ward off some of the side effects of the drug therapy, but some people have experienced life threatening results, especially diabetics. Check with your doctor first!

I have sjogren's and am taking plaquenil. Are there any exercises or anything I can do for the joint pain? Advil, motrin and tylenol (acetaminophen) don't work.

See details: How long have you been taking plaquenil (hydroxychloroquine)? It often takes 3-6 months to reach its maximum effectiveness. A low dose of Prednisone may help. Methotrexate is an option for longer term use.

Not safe: "usage of this drug during pregnancy should be avoided except when in the judgment of the physician the benefit outweighs the possible hazard. Please take note that this medication can rapidly be adsorbed by the placenta and my cause damage to the fetus's eyes.

What do you mean by: Safely? There have not been any drug-drug interaction studies with Linzess as yet. There is a risk of serotonergic syndrome with combining guaifenesin, phenylnephrine, Cymbalta (duloxetine) and Vyvance. Ambien is for occasional use. I would not combine it with Valium. Other than that you're fine.

What can substitute plaquenil (hydroxychloroquine)?

Nothing safer: Plaquenil is also known as hydroxychloroquine, so the parent drug Chloroquine can be substituted. But Chloroquine has significantly higher rate of retinal toxicity, so it isn't used much anymore. Also, Plaquenil appears to effectively reduce flares in patients with lupus not necessarily by suppressing immunity. So other meds like Imuran and CellCept (mycophenolate mofetil) also reduce flares, but also increases infection.

Most do fine: Most patients do very well on hydroxychloroquine. Common side effects include allergy to the medication, GI disturbances such as nausea or diarrhea, and changes of pigmentation in the skin or hair (this is not a complete list). You should also be tested for an enzyme called g6pd before starting, and you should have at least one eye exam annually while on the drug.

Depends: Often its use is long term but it depends on the manifestations of your sjogrens. The good news it is helping, as many people with sjogren's will discover, but consider the alternative that it might not work. It needs to be monitored, as does any medication.

Lupus Control: Lupus is a disease than can be serious and difficult to control. Forming a relationship with a rheumatologist is the best place to start.

As a lupus patient I have avoided foods like garlic and mushrooms (foods that improve immune system). Is it advisable? On 1g mmf bd, 400mg plaquenil (hydroxychloroquine)

Eat what you like.: I would not recommend any supplement advertised as an immune booster. It is ok to eat any fresh foods or vegetables. There is no research evidence that any specific diet or food will cause significant worsening or omprovement in sle.
