Soap & water: should be sufficient to clean under foreskin once it has been retracted fully. Phimosis, tight foreskin, could be cause of inability to easily retract foreskin which would make penile hygiene difficult. Need to see your Dr. or urologist if foreskin does not retract easily. Foreskin opening is physiologically narrow in most babies up to one year of age. Not abnormal up to and beyonf adge 3-years for foreskin not to retract completely, however the opening should be wide enough to prevent obstruction to urine flow.
