Inflammation, infection, tumors, trauma can cause such fluid. Abdominal diseases can produce abdominal fluid (ascites) that can track down into the pelvis (pouch of douglas) as well.

There are a number of reasons why, including sperm problems, not ovulating regularly, tube problems, etc. None of them is likely to be diagnosed by a scan. The lining of the abdomen, the peritoneum, produces fluid to lubricate all of the internal organs and naturally this fluid accumulates in the pelvis because were upright most of the time.
