What exactly is a cortisol stimulation test?

Cortisol Stim Test: A Cortisol stimulation tests should be performed when there is doubt about the status of hypothalamic-pituitary-adrenal function, and is used to establish the diagnosis in patients with equivocal serum cortisol values in whom hypoadrenalism is suspected. There are different types of test. Usually a low -dose ACTH stimulation test is done. Your doctor will chose the appropriate test for you. ...Read more

ACTH Stimulation Test results: baseline cortisol 6.2, 30 min 19.4, 1 hour 22.7. Suspect SAI?

ACTH Stimulation Test results: baseline cortisol 6.2, 30 min 19.4, 1 hour 22.7. Suspect SAI?

While there can be: variation between labs, these results appear to be largely normal. The baseline is normal (5-23 micrograms per deciliter). A rise from baseline of at least 7 to 10 ug/dl of cortisol, reaching at least 18 ug/dl at 60 minutes post-stimulation rules out primary adrenal insufficiency. A blunted response (not sure yours qualifies) may suggest secondary AI. Your Doctor will advise you. Stay well. ...Read more

My doctor ordered a cortrosyn stimulation test. Does this have to be done at 8am like my previos cortisol test or is time not a factor?

My doctor ordered a cortrosyn stimulation test. Does this have to be done at 8am like my previos cortisol test or is time not a factor?

It depends.: Usually, a Cortrosyn Stimulation Test give 4 results: ACTH(Baseline) & Cortisol(Baseline, 30', & 60'). The baseline ACTH & Cortisol are affected by time of day, and should be done as close as possible to your usual wake-up time (Traditionally ordered as 8AM). However, the most important values are the 30' & 60' Cortisols - these determine whether or not your adrenals can make enough Cortisol. ...Read more

Always so cold, low blood pressure 77/42 sometimes. Low blood sugar. ACTH stimulation test ok. But sometimes low cortisol. Ideas?

Always so cold, low blood pressure 77/42 sometimes. Low blood sugar. ACTH stimulation test ok. But sometimes low cortisol. Ideas?

Sounds adrenal: B"sd this is challenging - does sound like adrenal insufficiency - probably due to hypothalamic / pitiutary dsyfunction. The options are extensive work up, or limited work up with a trial of glucocorticoids and possibly mineralcorticoid - fludrocortisone. Steroid therapy has its own significant side effects, so needs to be expertly managed, with frequent attempts to wean off the steroids. ...Read more

Acth stimulation test results accurate?

Depends: I'm assuming no lab errors. If you get a very high cortisol spike after receiving the first dose of ACTH, you are reassured you do not have Addison's. No, or a very small spike, strongly suggests Addison's. In-between... that's why there are specialist endocrinologists. ...Read more
