My 3 yr old has had a headache and fever of 103.5 off tylenol (acetaminophen) and 100.5 with tylenol (acetaminophen) for the last 3 days and now on day 3 complaing of light sensitivy?

Headach & fever: How high is the fever? How severe is the headache? Any neck stiffness? Cough? Runny nose? Ear pain? Tylenol/advil/motrin (children's version, use appropriate dose for age and weight) will help both ha & fever. Shaking could be for a variety of reasons If he is alert and looking at you, he may have a "chill" or a "shiver" from the fever. You should be able to stop this movement. If he is having rhythmic shaking movements and unconsious, then it could be a seizure In either case he needs evaluation now.

Go to ED: My advice is similar- go the ed immediately, so that the appropriate tests can be done to reach a diagnosis. It is also important with respect to public health. If certain diagnoses are to be considered, then protective gear must be worn. Strep test?: Was a strep test done? Sometimes rashes and fevers are caused by viruses, and antibiotics don't work. Sometimes something that looks like scarlet fever can be caused by a similar germ called staph. If that's the case, a different antibiotic might be necessary. Whether the test was done or not, the persistence of symptoms despite antibiotics suggests the need for further investigation.
Need more info: Is the ha constant, do you have other symptoms, partic nausea, vomiting, sensitivity to light or sound, stiff neck, changes in consciousness, mental abilities. Do you have cough, chest pain, ear ache, other manifestations. Hopefully you will be better by the time you receive this, but if not, write back or go see your local doctor.
Best advice: Is to get yourself a comprehensive exam by your physician. There are many causes of that nature. Might be just a simple case of strep, sinus infection, allergy etc but you certainly don't want to miss out something more serious that can be treated at an earlier stage and save your life. Many: Your symptoms are extremely non specific. If the chest pain continues or worsens see a doctor immediately, especially if associated with difficulty breathing. You need to be sure you're not having a heart attack or something similar.
