Yes: While it is impossible to predict whether or not ovarian cysts/masses are benign or cancer, usually large ones need to be dealt with surgically. Rare: Ovarian cysts are of concern when over 5 cm. If the small cyst is not complex it is usually followed by ultrasound. Ca 125 has no diagnostic implication but is only of value for monitoring. Brca1 suggests the cyts should be followed. ...Read more

Follow up needed: Anechoic is an ultrasound term which means simple and usually benign. Hypoechoic is usually more complex and could be more worrisome. Your finding based on us and age are likely benign. However, because of the size you have an increased risk of ovarian torsion.
