Likely insignificant: This is relatively small. This is likely a "functional cyst" meaning present as part of the normal functioning of the ovary (like an egg follicle). Ovaries make cysts for a living! A follow up ultrasound in 4-6 weeks to confirm resolution is the best course of action.

Ovarian Follicles: Ovarian follicles especially small one are normal and should raise no concern. Ovarians make follicular cysts and luteal cysts which are normal.

Normal: Cysts of this size on ovaries are a normal finding on ultrasound. Functional cysts (i.e. Egg cysts or follicular cysts) are what ovaries make every month. Selection of one of these becomes the dominant egg which normally is released during ovulation.

Ovarian cyst: At 22, this is probably just a benign "simple follicular cyst", but if there are ultrasound suggestions of solid components, or blood flow increases by "color doppler" ultrasound exams, your provider may suggest laparoscopy for a tissue sample. See your provider for advice. Laparoscopy is an outpatient procedure, and will provide very accurate information.

Most likely: There is a very high probability that is what it is. Wouldn't worry too much, this is a common situation. It most likely will be gone on follow up. Was it 4 cm, not 4mm?

It's probably benign: The sonographic findings you describe are very suggestive of a benign ovarian cyst which is very likely to shrink on its own and resolve spontaneously over 2 to 3 months. If it does not, if it enlarges or if it is causing you pain and concerns, you may want to consult a skilled laparoscopic Gyn surgeon to discuss your options.

PCOS: Pcos stands for polycystic ovarian syndrome. This is one of the most common endocrine disorders in women. Women w pcos have abnormalities with the metabolism of androgens & estrogen. The tipical appearance of ovaries on ultrasound are prominent with multiple small follicular cyst. Clinical findings; menstrual disorders, infertility, hirsutism, obesity, diabetes, metabolic synd. Evaluation: It is not normal to bleed 25 days a month and the endometrial lining is expected to be thin since you are shedding the lining each time that you bleed. Once your obgyn will determine the underlying cause of your bleeding and your cycles will return to be normal then you can evaluate the thickness of the lining. The best time to evaluate the lining is prior to ovulation.

Re-evaluate: If the pressure is unbearable the safest option is reevaluation. Hopefully you had a urine culture sent and treatment is empiric at first. MD may consider augmenting your UTI therapy and ovarian cysts may cause torsion or there could be undetected pathology that needs reassessment. Need your full medical history, meds, age, vitals. Ovarian cyst: It will probably go away on its own. Have another ultrasound in 3 months. Go to an emergency room if you get worsening pelvic pain as it could mean the cyst has burst or your ovary has twisted. Birth control pills help to prevent cysts but you don't have to take them if you don't want to. For sure follow up in 3 months with your physician.
