Re: CFS+hypersomnia — I'm on modafinil & tricyclics. Am less hungry & sleep well, but still exhausted & not alert. Any other drugs effective for this?
Take Xanax (alprazolam) .5 mg & amitriptyline 12.5mg HS for sleep. Is it ok to take st johns wort in the morning to help with positive mood?
Get second opinion: The combination could readily backfire, as the Elavil (amitriptyline) and Ativan can both cause anti-cholinergic side effects, and additive sedation, lethargy, and dizziness. Both very low dose, but nonetheless may cause more problems than success. Not clear why these two have been started together. ?Psych Concierge appt? ...Read moreSee 1 more doctor answer
What could cause frequent pvcs (benign) daily? Bupropion XL 150mg 3x daily, Prozac (fluoxetine) 20mg, or the combo? Also take 200mg modafinil, levothroid 88mcg.
Your medications: The Modafinil is a stimulant which can cause abnormal hearth rhythms. Your levothroid, if not properly dosed could do it as well, ask your doctor for a blood test to make sure that dose is not too high for you. Being 58 years old, you could have heart problems unrelated to the medications. You should see a doctor to get this evaluated. ...Read more

12 years post op rny, iron 20 & vitamin d at 10. Seeing a new dr that hasn't worked with deficient gastric bypass, is 325ferrous 3x and 2000d 1x okay?
Nutrition!: A gastric bypass is a very complex nutrition situation. If your physician has not dealt with it much i would suggest asking for a referral to a nutritionist or a bariatric center. That being said, the dosage of iron is fda approved treatment for iron deficiency, and the vitamin d is a reasonable amount. You have many nutrient levels that should be monitored and carefully watched! ...Read more
I'm currently in Thailand. Is it possible to sub serlatine for citalopram, as they are both SSRI anti- depressants for anxiety.
Sure, for now.: You can make that substitution at a dose equivalent of 2.5:1. So, 25mg of Sertraline would be 10mg of Citalopram. 50mg of Sertraline would be 20mg of Citalopram, and 100mg of Sertraline would be 40mg of Citalopram. Don't take more than 40mg of Citalopram. And you can return to Sertraline upon your return. ...Read more

I take a multivitamin plus vitamin c, d, all b's with niacin and fish oil, calcium, red yeast rice, should I take the vitamin magnesium with these & why?
I don't like it.: Truth be told, I don't like combining Niacin with the red yeast rice as they can both have toxic effects upon the liver. I hope you're having your liver blood enzyme levels monitored on that combination. Adding Magnesium to all of this is not my concern as I don't see any issue in that as long as your doses of all these other medications are not excessive. ...Read moreSee 2 more doctor answers

Any tip make a prednisone taper below 5 mg less taxing? I'm REALY struggling & it's affecting my job. I told my doctor & he just said keep tapering.
Prednisone Taper: Unfortunately, some of us don't really listen or understand what our patients are telling us. Look Doc, I can't function at all with my prednisone under 5 mg. I don't know how long or why you are on prednisone. If you can't make him listen and it is affecting you like that, then maybe you need a second opinion. That's how I would handle it. ...Read more
