Could be a cyst: A cyst, neuroma, or hypertrophic scar could be causing pressure on on sensory nerves in the area of the scar with exertion (exercise). After 5 years, your scarring is mature, so if you are having persistent symptoms, you may want to consider a scar excision/revision to hopefully improve/relieve your intermittent pains (typical of nerve pain).
