Hard or pellet-like: stools are a sign of constipation. If an infant of 6wks. or younger strains to pass soft stool, it can be normal. The smooth muscle of the intestines is not yet coordinated to propel its contents, so they may draw up their legs, grunt, turn red in the face & use voluntary abdominal muscles to assist. Solely breast-fed babies hardly ever have hard stools; adding formula can make them harder. ...Read more

Reflux: It can be normal for a 3 week old to spit up. As long as pt is not fussy with spitting and pt is gaining weight this can be normal. This is probably gastroesophageal reflux. Tends to worsen after about a month of age. You can keep pt in an elevated position after feedings for 30 minutes. Breastfed babies tend to spit up less.
