Can ACTH test rule out Cushing's? ACTH stim. Cortisol levels: T0: 9.1 T30: 16.6 and T60min: 18.6. Should I have 24 hr cortisol levels checked?
Wrong test: Hi. The ACTH-stim test is not a test for Cushing's. It's a test for adrenal insufficiency, more exactly, PRIMARY adrenal insufficiency. So no, an ACTH-stim test cannot rule out Cushing's. Someone needs to suspect you of Cushing's to order the 24h urine cortisol. We don't have any info to make us suspect Cushing's in you, so we can't say if you SHOULD have a 24h urine cortisol. Your ACTH-stim is nl ...Read more

Total protein, 24 hr urine
is this good or bad?
(protein, urine, qn <=11 mg/dl -my result <6)
protein, 24 hr urine -unable to calculate. A1c, kidney panel, ua, lipid normal
Urine results: Your urine shows that you have virtually no protein in your urine, which is a good thing. Patients with proteinuria have an increased rate of kidney failure, heart attacks and strokes which is related to the amount of protein in the urine both in diabetic and non-diabetic patients. What was the reason for the urine collection? ...Read moreSee 1 more doctor answer
24 hours urinary free cortisol level(3 samples) is high, but acth (blood)is normal.Have I got Cushing's syndrome or these values depends to my obesity?
Depends on the level: but it's a reason to get a simple ultrasound to look at adrenals and ovaries. (a slight elevation of the cortisol is not very specific and while correlating with obesity which causes which gets ...tricky. A BIG elevation of it pretty much has to be from overproduction by the adrenals). ...Read moreSee 1 more doctor answer
How could free testosterone measured by equilibrium dialysis (mayo labs) be over 50% more than calculated free testosterone (issam.Ch)? Data (4/26): tt 533, shbg 41, albumin 4.5, cft 9.7, ft 15
Lab variation: The calculated free t will depend on how accurately the lab measures each component (total t, shbg, albumin) as well as the algorithm used to do the calculation. The free t by dialysis can be more accurate, as it depends on one test being done correctly. However, labs vary enormously in how well and accurately they can do the test. If the levels are normal, don't fret the details. ...Read moreSee 1 more doctor answer

43 yro f tested high for cortisol in afternoon 28 ug/dl After suppression test am cortisol 3.7 and ACTH 2.0 CT showed adrenals normal. Ideas?
Cushing's diagnosis: Presumably you have symptoms (weight gain, skin changes, diabetes, etc). Sounds like you had screening tests (?overnight dex) but these tests do not establish the diagnosis. False positives occur with depression, obesity, alcohol consumption. You really need to see an Endocrinologist to get this worked up. If it is Cushing's, 70% is in the pituitary, not the adrenal. ...Read more
Doc order a Cortisol free 24 hour urine collection and a metaneprine Fractionated. Are those two separate reports?
Which of the following accurately predicts problems with cortisol levels blood test, 24 hr urine or midnight saliva swab?
Depends: Blood cortisol is useful in the diagnosis of cushings (hypercortisolemia) or adrenal insufficiency. A 24 hr urine cortisol is helpful to diagnose excess cortisol. Saliva cortisol is only helpful in diagnosing excess cortisol, not adrenal insufficiency. If a blood cortisol level is low, further evaluation is required to establish if your adrenal function is truly low. ...Read more
Acth stim test.My baseline cortisol was 17.5. After 30 min, cortisol =22. After 60 min, =26.I thought cortisol should raise 20pts but i was told normal.?
Perhaps: In healthy individuals, the morning cortisol level should double from a baseline of 20-30 µg/dl within 60 minutes. The baseline cortisol level in people with adrenal insufficiency is usually below 15 µg/dl. An acth stimulation test that raises the cortisol concentration to 20 µg/dl would support the diagnosis. Make sure you have the right units. ...Read more

Acth stim test done. Acth was 12 at baseline cortisol 1. At 30 minutes cortisol was 22.7 then 1hr at 29. Does this indicate secondary ai?
Adrenal insuff: The results of the acth stim test suggest that ai is not an issue. The cortisol level responded as expected to the stimulation. If at the start of the test, you were ill then it might suggest that you have a relative ai as your initial cortisol level was 1 (ok if your healthy and not stressed, but in an ill person the cortisol should be higher in response to stress). ...Read more

No clear relation: There is no clear relationship known between bipolar disorder and cortisol levels. While a number of studies have shown a correlation between cortisol levels and mood disorders these studies are not always conclusive and not always replicated. ...Read moreSee 1 more doctor answer
