OK unless recurring: Even if it is blood, a small amount, one event would not be anything to worry about. It could be due to the thermometer causing some mild trauma or constipation. Various things: Bright red blood can be an anal fissure. Blood mixed throughout stool warrants a visit to the doctor. Bloody loose stool could be salmonella or similar bacterial infection. Occult blood test: Fecal occult blood test is done to see if you have evidence of bleeding in the GI tract. The sensitivity will be very low if it is done only once. The sensitivity will be much higher if that is done in 3 separate occasion. Soften the stools: What else is going on? Is there fever or abdominal pain? Diarrhea? Most likely etiology can be constipation. Pt should try to drink more water and try to change diet to soften the stools. If bleeding persists despite resolution of constipation need to consider other etiologies including hemorrhoids or other vascular anomalies or potentially a benign growth. Need GI consult at this point.

NO blood: No blood should be in the stool If the child is constipated then this may explain it. BUT. The child needs seen by the doctor and then problem reevaluated. If the child has ongoing clay colored stool this will need to be looked into. Take the child to the pediatrician Best wishes.
Need evaluation: Blood in stool should be evaluated, unless it is scant and follows after a large, constipated stool, which is likely to give a small tear or fissure. Short term weight loss in a 5 year old can occur with acute illness like the flu, but if accompanied by blood in the stool, it implies some type of colitis, and this should be evaluated.
