US: First trimester ultrasound is based on biometric measurements only, the embryonic crown-rump length, which is extremely reliable when done by a skilled sonographer. No assumptiosn need be made at all. Possibly.: Typically the pregnancy grows in the path of least resistance - i.e. Inside the uterus. Most likely everything will be fine; if you or your obstetrician are concerned about this, an MRI should help put everyone's mind at ease by imaging the entire pelvis and uterus in detail. I would: Usually we don't worry until we can't hear it at 12 weeks. But in a worried woman with a prior history of miscarriage, i would order an ultrasound out of kindness. ...Read more

I Hope Not!: Twins at 7 weeks should be fairly clearly visible on a transvaginal sono. As far as whether or not they might be identical, that is a more difficult question to answer by sono and requires careful analysis of the membranes surrounding fetuses, unless there is a single sac which would only be seen in an identical twins (rare). Not sure: Can't explain why someone would or wouldn't without being there. Maybe it wasn't as clear cut, maybe they wanted a followup. Maybe they didn't know.... ...Read more
