Family Doctor Exam!: Your problem obtaining or maintaining an erection with low self-esteem & sex drive could be due to psychological issues or medical illnesses (such as heart disease or diabetes) or both. It is for this reason that I encourage you to see a Family Doctor for a medical exam. A Family Doctor is well qualified to also discuss sexual health issues with you. I wish you well with your health!

Sexual Issues: I agree with Dr. Howard Mandel. In addition: First have a complete physical exam by a Dr. to rule out underlying biological problem. If no problem then consult with a Clinical Psychologist for psychotherapy help and a Psychiatrist for medication consult.
