See MD: Unfortunately, there is about a 1 in 300 chance that a tubal ligation fails. Do a pregnancy test to be sure. If it's positive, there is a chance that you have a tubal pregnancy, get help immediately. If negative, you may have a hormonal imbalance causing your uterine lining to thicken. See a provider to sort this out.

Complication of tub: any woman of reproductive age with pain and bleeding particularly with a history of tubal surgery needs evaluation to rule out an ectopic pregnancy .This will generally consist of pregnancy hormone testing and correlation with timed ultrasound examination. It may be just menstrual cramping(dysmenorrhea) but you should not make that assumption without interaction with a gynecologist.
