Bizarre question: How would you have sex in a space no more than a few millimeters in diameter. If it could happen , sperm could still spill into the vaginal chamber & reach the cervix to start a pregnancy.

Very small penis: How you could conceivably have sex through the urethra is unimaginable. Attempts to do this however may deposit semen containing sperm onto the perineal surface and they can find their way into the vagina and create pregnancy.
