Likely: Sources hemorrhoid or fissure. There are over the counter remedies, if persist, you may need evaluation for other more serious causes.

Fissure: This could all be due to an anal fissure that hurts and bleeds with a bm. Another possibility is a thrombosed hemorrhoid. Get examined and in the meantime apply warm soaks and use glycerin suppositories to lubricate the anal region. Take a stool softener so that stool won't be too hard.

Rectal bleeding: This is most often from hemorrhoids in the rectal area, that can come from various problems, usually constipation. If constipated get on regimen of whole fruits and vegetables with addition of fiber foods, oatmeal, bran and nuts.
