Many possible causes
Muscle strains, pneumonia in the l lower lobe, spleen inflammation, constipation, gas cramps in the colon, etc. See you doc for furhter evaluation.
Severe cough and undue exertion like lifting heavy stuff can cause such symptoms.
It could be your diaphragm. Make sure you stretch your trunk muscles routinely, it could help.
I am sorry to hear of your pain. The differential diagnosis includes scoliosis with muscle spasm along the convexity of the curve, thoracic nerve root injury, or retroperitoneal problems such as kidney stones.
