Not one drug: It depends on how you developed the back pain but in general physical therapy and non-narcotic medications work best.

No one best: There is no one best narcotic for your pain. Choice depends on many factors such as coexisting diseases, side effects, initial diagnoses, breakthrough pain, patient compliance, cost... Discuss your options with your pain management physician.
