how can you tell the difference between perioral dermatitis and herpes around the mouth?

Lotion: Dermatitis usual resolves with lotion. Herpetic lesions usual recur in the same region and does not improve with lotion. Herpes lesions improve with antiviral medication.

And the antiviral medication should be orally administered rather than in a topical preparation.
