how difficult is it to attain an aesthetic look when you have narrow shoulders?

Posture/muscle/waist: You have not indicated if you are make or female which may effect the ultimate proportions. Whereas women are generally judged by their waist to hip ratio, men are assessed by their shoulder to waist ratio. A large waist can make narrow shoulders look even smaller and vice versa. Posture and shoulder rotation is also key. Lastly, work on deltoid muscles to increase width and bulk.

Narrow shoulders: Consider weight lifting to build muscle mass and help with posture. A crossfit gym or olympic weightlifting gym is a good starting point.
