A 59-year-old male asked:

Dental implant: what system is available that uses flap surgery that can then be sewn directly up around & against the implant vs. an open gum wound?

Almost all of them: All tooth-form dental implants, with the exception of the "1-piece" implants can be treated in a single stage abutment portion exposed through the gums) or two-stage where the gum tissues are treated to heal over the implant. The dentist will determine which approach is best for you based on many factors: amount of gum tissue, bone width, height and quality. New studies actually support "open.".

Too many to list: Two stage means first opening the gum, placing the implant and then a second procedure in 8-12 weeks to uncover the implant to attache a healing abutment. That sound like what you want versus either an implant that sticks up through the gum or placing an abutment at the time of surgery. Discuss your options with your dentist.

Implant : Seek a dentist that works with an oral surgeon and discuss your options. I recommend placing the implant under the gum to adapt and then place healing cap followed by your final crown.

All systems: Nowadays, the dental implants systems are advanced where the gum flap can be sutured around the implant. Sometimes, a flap may not be required. The more important issue is that the dental implant heals well in the jaw bone and functions well.

All systems...: Implant can be placed as submerged (gum suture on top), non submerged (gum snug up to the healing abutment), or immediate restored (tooth restored immediately following implantation). These are technique sensitive and can be done with all good implant system. Submerged is prefer since there is a least amount of micromovement and least chance of external factor leads to infection of the implant.
