Level of risk: Vaccines are recommended for people at risk for certain diseases, most take 2-4 weeks to work. Military personnel are at far greater risk, due to the nature of their work, for certain diseases. Thusly they are immunized against diseases we don't experience in the USA. This is particularly true for personnel on 'mobility' ie mobilized to the far reaches of the planet on a moment's notice.

All vaccines are available to other than military people if they meet certain criteria. Some include Civil Service or other government employees who must, by the nature of their work, be in areas where specific vaccines have to be given. Certain countries have their own requirements to enter. In that case a person may get a vaccine not usually given to the public. Some are risky but necessary.

With treatment, the death rate drops from about 50% to about 15% (the majority of the cases are bubonic plague). Pneumonic plague is fatal if treatment is not started in 24 hours.

Surgery ussually: Colon obstruction can be caused by benign conditions(diverticulitis, volvulus or twisting, constipation, inflamatory bowel didease) or cancer. Sometimes medications or minor procedures can help( atleast temporarily) , surgery is usually required for majority of causes, especially when cancer is associated.
