my shoulder hurts a lot and the collar bone on the hurt side sticks up or out more than the other side. it also hurts to lift pull or put pressure onit?

Separated Shoulder: This may be something known as a separated shoulder (medically known as an ac joint separation). Most of these injuries are treated non-operatively with immobilization, therapy antiinflammatories and injections. If very painful, would recommend a visit to a sports medicine or orthopedic specialist.

Ac separation is likely if there has been an injury. If no trauma, then ac arthritis/hypertrophy is a possibility...depending on your age.

Sounds like you may:: Have had ac joint separation (joint between collar bone {clavicle} and acromion {part of scapula-- shoulder blade}), a DX which fits very well with your symptoms. Its due to tearing off the ligaments holding the 2 bones together. See your orthopod to start treatment.
