A 39-year-old female asked:

Is there any medication that can curb sociopathic behavior in a 9 year old with bipolar, rad, and adhd who has not responded to 6 years of therapy?

Start with work-up: No medication currently available treats sociopathy, but that is a quite rare condition at age 9, and you mention a number of other diagnoses that potentially are medication responsive. You did not state what you meant by "therapy" or whether the child has been on medications already. I recommend going to a strong academic center specializing in children and obtaining a thorough assessment.

Environment: I would focus as much as possible on environmental enrichment as well as cautious pharmacotherapy. I think a child with anger can become a sociopath. There is no medicine that is specific for sociopathy although there are medicines that can reduce impulsivity and aggression. It could be argued that sociopathic behavior is as related to psychodynamic variables as well as biological predisposition.

A 9-year-old with: Reactive Attachment Disorder, genetic loading for early-onset ADHD & Juvenile-Onset Bipolar Disorder & early adverse life events shaping his personality is probably an adopted child. Add a Developmental/Behavioral Pediatrcian to his team to investigate underlying genetic, biological & environmental factors; e.g., Fetal Alcohol Spectrum Disorder, Fetal Drug Effects, syndromes like VCF, etc.
