? Inflammation : Sudden testicular pain in the absence of trauma should be aggressively investigated if the pain persists for beyond a short time. Torsion(twisting) of the testicle is very painful and is a surgical emergency if the testicle is to be possibly saved. Epididymidis or inflammation of the epididymis is another cause that might respond to antibiotics. He should see a urologist as soon as possible.
