PCP: This drug is variable, from mild hallucinations with small doses to agitated psychosis with larger doses and bad reactions. This could lead to a fatal accident. Many pcp users end up in the emergency room with consequences from their drug use.

Several: Pcp is a term for pneumocystis carinni now call p. Jirovessi. Usual therapy requires septra (sulfamethoxazole and trimethoprim) although other special antibiotics are useful in case patient is allergic or resistnt to sulfas. Important to know that severe infections required concomitant steroids.

ID doc consult...: Standard therapy for pcp (p. Jirovecii) is trimethoprim-sulfamethoxazole (tmp-smx). Depending on the degree of infection, Prednisone may be used, to control inflammation. People with sulfa allergies can alternatively receive Clindamycin + Primaquine or Atovaquone (mepron). Ensure that a "g-6-pd test" is done and is normal, or else a rapidly developing anemia can ensue with the use of tmp-smx.

PCP tx: Agree with other doctors. Aerosolized pentamidine is also used for prophylaxis and treatment of pcp.

Patient: Generally patient is the one who gives the history. But any other information like medications etc , we may have to get the information from the pcp.

History-taking.: Three ways at least. (1) Ask you, the patient, and/or members of your family. (2) Access your hospital chart. (3) Access your regular doctor’s chart - hospitals and outside offices these days often will share the same electronic medical record, so a Hospitalist can access your hospital chart, office chart, blood test results and radiology test results directly.
