Several things: There are whole chapters in gynecology books for GYN MD's that are exclusively devoted to abnormal menstruation and its' causes. See a GYN MD for an appointment who has read the chapters in those books, taken and passed the tests(Board certification), and can begin a detective analysis of the problem and maybe a "solution".

Need Gynecology Eval: Vaginal bleeding is not always your menstrual peiod. Sometimes women get miscarriages that happen about the time that they expect their periods. Also, there are a lot of conditions that can develop in the uterus that can cause bleeding such as fibroids, uterine polyps, cancer, infections, etc. Your sister should go and see her pcp or a gynecologist for an evaluation of her vaginal bleeding.

Dr. Trachella Johnson foy

Age, Hormones, tumor: Most common causes of irregular menses would be dysfunctional uterine bleeding and/or anovulatory cycles. Fibroids (benign tumors in uterus), or overgrowth of uterine lining ( benign or cancerous) can cause irreg bleeding as well. At 44 hormonal changes & perimenopause are likely causes. Ultrasound & labs can be done to evaluate for causes. Hormonal regulation or uterine procedures can be done.
