Tonsillitis: You may have tonsillitis caused by infectious mononucleosis since your culture was negative and you have not responded to azithromycin. Blood tests called cbc and differential, liver function tests as well as a monotest should be done to check for infectious mononucleosis which can also cause liver inflammation and enlargement of spleen and lymph nodes and Marked fatigue is common.
