Not enough lube: If the issue is actual cuts on the penis, it may be related to not using enough lubricant, however it can also be related to inflammation or irritation caused by an undiagnosed STD. I would recommend that you see a doctor and get tested.

Other possible cause: I would also check for retained foreign bodies in the receptive area, so, both partners should be checked.
