When: You develop chest pain and symptoms of a heart attack but no muscle dies. It could have just been angina without actual cardiac injury. Cardiac lab tests ( enzymes) would be normal.

Not usually: Blue/ purple nail beds are a serious sign of poor circulation.
Ischemic tissue with poor flow, or venous congestion and extraction of oxygen due to stasis.
Need diagnosis and treatment.
